Travel RN Care Coordinator needed for a Post–Heart Transplant
ambulatory program. The RN will coordinate care across the
continuum for post–heart transplant patients, monitoring graft
function, immunosuppression therapy, infection risk, and lab
results while collaborating closely with physicians, APPs,
pharmacy, and the multidisciplinary team. Responsibilities -
Coordinate post–heart transplant care per protocols and provider
orders - Monitor immunosuppression, prophylactic antibiotics,
rejection, and infection - Review labs frequently and escalate
changes requiring immediate intervention - Collaborate with MDs,
APPs, PharmD, and consulting services - Educate patients and
families and coordinate support services Requirements - Active
California RN license - BLS (AHA) - 1 year ambulatory care
experience - Cardiac nursing experience with heart failure and/or
heart transplant patients - Epic EMR experience • Medical & Health
