This is a full-time, 40 hour/week position based out of the St. James Health Center located in San Jose, California. The Population Health Manager is responsible for planning, implementing, and evaluating population health initiatives that improve patient outcomes, reduce health disparities, and enhance the overall quality of care across Gardner Health Services. This role supports data-driven care coordination, preventive health, and chronic disease management programs in alignment with HRSA, NCQA, and value-based care standards. Minimum Qualifications: The minimum qualifications of these positions are listed below. An equivalent combination of education and experience may be considered. Education & Experience: Bachelor’s degree in Nursing, Public Health, Health Administration, or related field required; Master’s preferred. Minimum 3–5 years’ experience in population health, quality improvement, or care coordination—preferably in an FQHC or community health setting. Experience with value-based care models, risk stratification, and chronic disease management. Knowledge, Skills, and Abilities: Strong understanding of HRSA clinical performance measures, UDS reporting, and PCMH standards. Familiarity with EHR data extraction, reporting tools, and quality dashboards. Excellent leadership, communication, and analytical skills. Ability to collaborate effectively across multidisciplinary teams. *If selected, other documents and certifications may be required as part of the hiring and credentialing process* Compensation: Compensation and level of position placement will be determined based on relevant experience and education.
