Physical Therapist Location: Sunnyvale, CA Agency: Cynet Health Pay: $2,850 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 3/23/2026 About the Position Job Title: Physical Therapist Profession: Rehabilitation Therapy Specialty: Physical Therapist Duration: 13 weeks Shift: Days Hours per Shift: 8:00 am - 5:30 pm (Schedule can be flexible based on patient's availability) Experience: Minimum of 1 year of Home Health experience License: Current state license as a Physical Therapist Certifications: BLS/CPR certification preferred Must-Have: - One year of Home Health experience Description: The Physical Therapist's primary goal is to restore function to prevent or minimize disability following disease or injury. They help clients reach their maximum performance/function within their capabilities. The therapist evaluates and treats patients using medically prescribed physical therapy programs. They may supervise Physical Therapy Assistants and make initial admission/evaluation visits. The therapist notifies the assigned Clinical Team Manager of OASIS scores within established timelines. The ideal candidate documents observations, clinical findings, problems, skilled interventions/treatments, goals, and discharge plans. This individual also performs other duties as assigned consistent with their skills and training in alignment with the organization's goals and mission.
